Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding or collapse: a) Warehouse - On or about November 1, 2012, on the north side of the builiding in the box aisle a bale of corrugated paper stacked on top of another bale tipped over and fell on an employee who was loading bundles.
