29 CFR 1910.36(g)(2): An exit access must be at least 28 inches (71.1 cm) wide at all points. Where there is only one exit access leading to an exit or exit discharge, the width of the exit and exit discharge must be at least equal to the width of the exit access.    In the event of an unforeseen emergency such as a fire or release of chemicals, employees were exposed to asphyxiation and fire hazards in that adequate egress was not provided in the following areas:     a) On the Slaughter floor, at the East side of the head table area, the employees from first shift and second shift that remove pins were allowed approximately 23 ? inches for egress.    b) On the Slaughter floor at the West side of the head-table, the employees from first shift and second shift were allowed approximately 19 ? inches of egress, after descending down a set of stairs of 5 steps.    c) On the Slaughter floor on the West side of the head-table employees from first shift and second shift are allowed approximately 18 ? inches of egress, after passing through a 19 ? inches of egress from the head-table area.     d) On the fab-floor area the employee known as a Sorter at the sorting area uses a small open space, as a means to enter and to egress from a working platform, with measurements of approximately one foot wide and approximately three feet in height.    e) On the Fab-floor, the employee known as a 2nd sorter in the sorter area is allowed an egress underneath a conveyor belt of approximately 38 inches in height.    f) On the Fab-floor, the employees working at the trim-trim table area are allowed a passageway of approximately 13 inches for egress.

29 CFR 1910.37(b)(1): Each exit route must be adequately lighted so that an employee with normal vision can see along the exit route.    On the Slaughter floor, employees using knives were exposed to trips, falls and stuck-by knife hazards, and in the coolers employees were exposed to trips, and fall hazards. The employees were exposed in that illumination for egress in the event of an unforeseen power outage was not available in the following areas:    a) In the offal room approximately 50 employees from 1st and 2nd shift were exposed.  b) On the Slaughter floor in the skin-pizzle room approximately 2 employees from 1st and 2nd shift were exposed.  c) On the Slaughter floor, in the head-flush area approximately 10 employees from 1st and 2nd shift were exposed.  d) On the Slaughter floor, in the head-drop area approximately 8 employees from 1st and 2nd shift were exposed.  e) On the Slaughter floor, on a working platform (Skinner-bench) area approximately 18 employees from 1st and 2nd shift were exposed.  f) On the Slaughter floor, in the up-puller area approximately 2 employees from 1st and 2nd shift were exposed.  g) On the Slaughter floor, in the sticker area approximately 4 employees from 1st and 2nd shift were exposed.  h) On the Slaughter floor, in the knocker area approximately 4 employees from 1st and 2nd shift were exposed.  i) On the Slaughter floor, in the shackler area approximately 4 employees from 1st and 2nd shift were exposed.  j) On the Slaughter floor, in the cattle wash area approximately 2 employees from 1st and 2nd shift were exposed.  k) In the Hot-box area, approximately 4 employees from 1st and 2nd shift were exposed.  l) In the cooler at the break chain area approximately 8 employees from 1st and 2nd shift were exposed.  m) In the cooler in sales area approximately 18 employees from 1st and 2nd shift were exposed.

29 CFR 1910.37(b)(2): Each exit must be clearly visible and marked by a sign reading "Exit."    In the Offal room next to the Slaughter floor the exit was not marked with a sign reading exit.
