29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.  On or about November 01, 2012, employees were working from a welded frame scaffold and were not provided with hard hats while other employees were working above them, exposing employees to the hazard of being struck by falling tools or materials.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are provided and utilizing protective helmets where overhead hazards exist.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  On or about November 01, 2012, employees were working from two welded frame scaffolds that are two tiers in height and 5 sections in length where the work platforms were not fully planked or decked, exposing employees to the hazard of 12 foot falls.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each platform on all working levels of scaffolds are fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Cross braces shall not be used as a means of access.  On or about November 01, 2012, employees were working from two welded frame scaffolds which measured 2 tiers in height and 5 sections in length and both scaffold were not provide with a ladder for access and egress to and from the scaffold, exposing employees to the hazard of 12 foot falls.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that ladders are provided to employees accessing scaffolding.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about November 01, 2012, employees were working from two welded frame scaffold which were 2 tiers in height and 5 sections in length, one on the front, East side, of the building which was not provided with guard railing at all and the other on the North side of the building which had guard railing, but was missing on one section of the scaffold, exposing employees to the hazard of 12 foot falls.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees on scaffolds more than 10 feet above a lower level are protected from falling to that lower level by fall protection.
