29 CFR 1910.133(a)(3):     The employer shall ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.    a.  At the workplace, the employer did not ensure employees with prescription lenses wore adequate eye protection exposing employees to potential eye injuries.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    a.  At the workplace, in machine area one, a Wellsaw mini bandsaw, model 58B, serial number 15406 had approximately two inches of the unused portion of the sawblade unguarded exposing employees to an amputation hazard.

29 CFR 1910.215(a)(1):     Machine guarding. Abrasive wheels shall be used only on machines provided with safety guards.    a.  At the workplace, next to the 3301 Mazak CNC Lathe, a Baldor pedestal grinding machine had an abrasive wheel that was not guarded exposing employees to a struck-by hazard.    b.  At the workplace, in Machine Area 1, a Kingston Conley Electric Company pedestal grinding machine had two abrasive wheels that were not guarded exposing employees to a struck-by hazard.    c.  At the workplace, in the Tool Area, a pedestal grinding machine had an abrasive wheel that was not guarded exposing employees to a struck-by hazard.    d.  At the workplace, in the Open Mill Area, a Baldor pedestal grinding machine had two abrasive wheels that were not guarded exposing employees to a struck-by hazard.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.    a. In the open mill area, a Tecknics CNC Mill, Model RC-724-EX, Serial Number 88960419, had a flexible cord connected to a Fanuk motor that was not connected to a device and fitting so that strain relief was provided.    b.  In the tool area, a manual Bridgeport Textron, Serial Number J223450, had an outlet box serviced with a flexible cord that was pulled out of its coupling putting strain on its terminals.
