29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a. At the site - an employee was exposed to an approximate 15 ft fall hazard, in that, fall protection was not worn properly. A 6 ft lanyard was attached to the side d-ring instead of the back d-ring. Violation observed on or about 11/1/12.
