29 CFR 1910.254(d)(7):     Electrode holders when not in use were not so placed that they could not make electrical contact with persons, conducting objects, fuel or compressed gas tanks:      On or about November 1, 2012, in the main shop, employees performed single hand welding on a skid and were exposed to electric shock and impalement hazards from a welding rod left in a stinger and laying on the ground in the walking/working area.

29 CFR 1910.303(f)(2):     Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:    On or about November 1, 2012, in the main shop, employees operate flexible cord and plug tools and equipment during fabrication of oilfield skids and the circuit breakers in electrical panels were not identified such as but not limited to the following locations:    a)  Square D QO Load Center for the middle shop with five circuit breakers;     b)  Square D QO Load Center for the lighting with thirty-seven circuit breakers;    c)  Square D Rainproof Load Center at outside electric panel board with ten circuit breakers.

29 CFR 1910.305(b)(1)(ii):     Unused openings in boxes, cabinets, or fittings were not effectively closed:    On November 1, 2012, at this establishment, employees used flexible cord and plug hand tools during the fabrication of oilfield skids and the electric panel boards had unused openings where energized conductors were exposed such as but not limited to:    a)  In the main shop, the Square D QO Load Center for the middle shop had one missing circuit breaker;    b)  In the outside yard, the Square D Rainproof Center had one missing circuit breaker;    c)  In the outside yard, the Square D Rainproof Center had one unused opening at the bottom of the panel where a conductor was missing.
