29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) On or about November 1, 2012 at 3521 E 57 th Kansas City, Missouri 64130, workers were exposed to a hazard from working on a pitched roof. 8 employees working at elevation (approximately 18 feet) without fall protection.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder. (a) On or about November 1, 2012, at 3521 E 57th St Kansas City Missouri 64130. Workers were exposed to a fall hazard, as they were ascending and descending a ladder on the south side of the apartment building . Employees were observed using a portable extension ladder that was not extended at least 3 feet above the upper landing surface.
