29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a)Reusel Construction, LLC @ 2605 W. 144th Ave., Broomfield, CO 80020: On and before October 22nd, 2012, the employer did not ensure entry and egress form the trench was provided. Employees were accessing and egressing by climbing in and out of the excavation without the use of a ladder. The excavation was approximately 8 feet in depth. This condition exposed employees to the hazard of trench cave-in and fall hazards.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraphs (b) or (c) of this section: (a)Reusel Construction, LLC @ 2605 W. 144th Ave., Broomfield, CO 80020: On and before October 22nd, 2012, the exposing employer did not ensure that each employee conducting work in the excavation was protected from cave-in, in that the excavation was approximately 8' deep on the north wall, 8' on the south wall, and 4 feet in width, and lacked any protective system, such as appropriate sloping, benching, or shield system. This condition exposed the employees to a crushing hazard by excavation wall cave-in.
