29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) Integrity Iron, LLC located at 3045 West Prospect Road in Fort Collins, CO 80526: On and before October 17, 2012, two employees were placing and tack welding parapet tube steel to supporting columns while in a JLG 400S aerial lift and were not wearing fall protection. This hazard exposed the employees to a fall of approximately 16-17 feet.

29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper landing, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing, and was not secured: (a) Integrity Iron, LLC located at 3045 West Prospect Road in Fort Collins, CO 80526: On and before October 17, 2012, the employer did not ensure that a portable extension ladder, used to access a horizontal beam at a height of approximately 10 feet, extended 3 feet above the upper landing, or was secured. Condition exposed 1 employee to a fall hazard of approximately 10 feet.
