29 CFR 1910.272(k): Grate openings. Receiving-pit feed openings, such as truck or railcar receiving-pits, shall be covered by grates. The width of openings in the grates shall be a maximum of 2 1/2 inches (6.35 cm). In the Truck Receiving/Unloading Area #2, one of metal grate openings for the auger measured 2 3/4 inches.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. In the Maintenance Department, maintenance employees were using extension cord sets attached to a 220 Volt AC receptable outlet box with knockouts, in violation of its UL Listing/Labeling for the receptacle outlet box.
