29 CFR 1910.178(a)(4): Modifications and additions which affected capacity and safe operation were performed by the customer or user without manufacturers written approval, and the capacity, operation and maintenance instruction plates, tags or decals were not changed accordingly: A clamp attachment and a turntable attachment were used with various forklifts to move containers of scrap metal in the facility. No written approval from the forklift manufacturer was obtained by the employer and the attachments were not reflected on the data plate on the forklifts.
