29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about November 2, 2012 and times prior thereto, employees of Land O' Frost, Inc. performed maintenance activities at the facility located at 911 Hastings Ave. in Searcy, Arkansas.  Machinery used by maintenance employees were observed unguarded, exposing employees to amputation hazards in the following instances:    A.  A drill press (model number: 15-655 and serial number: 1800529) manufactured by Rockwell was observed in the Maintenance Department of the Packaging Area.  The drill press was not guarded at the point of operation to protect employees from the rotating chuck and swarf produced by the drill bit when operated.    B.  An EXTRAK II milling machine manufactured by Bridgeport Machines, Inc. was observed in the Maintenance Department of the Packaging Area.  The milling machine was not guarded at the point of operation to protect employees from the rotating cutter while operating.    C.  An EXTRAK I milling machine manufactured by Bridgeport Machines, Inc. was observed in the Maintenance Department of the Packaging Area.  The milling machine was not guarded at the point of operation to protect employees from the rotating cutter while operating.    D.  A 15 inch variable speed drill press (model number: 2221VS and serial number: 2108104) manufactured by Wilton was observed in the Maintenance Department of the Processing Area.  The drill press was not guarded at the point of operation to protect employees from the rotating chuck and swarf produced by the drill bit when operated.    E.  A 100 ton hydraulic shop press manufactured by OTC was observed in the Maintenance Department of the Processing Area and was used by employees when repairing bearings for machinery.  The hydraulic press was not guarded at the point of operation, exposing employees to amputation hazards.
