On or about November 2, 2012, at the west side of the building, the employer was using a National articulating crane, Series 4, to lift materials to the roof and did not have written documentation to prove that a monthly inspection had been completed, including the date and signature of the person who did the inspection, exposing employees to physical injuries in the event of an equipment failure.
