29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or about 11/2/2012, employees were working on an unprotected edge while conducting construction activities and were not protected from fall hazards.    Instance 1: An employee standing close to the edge of a small commercial roof was exposed to a 13.3 foot fall hazard while giving hand signals to a lull operator during the hoisting of building material onto the roof.    Instance 2: Three employees were exposed to a 10.5 foot fall hazard while demolishing existing commercial roof.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about 11/2/2012, the employer did not provide fall prevention training to employees exposed to fall hazards.     Instance 1: An employee standing close to the edge of a small commercial roof was exposed to a 13.3 foot fall hazard while giving hand signals to a lull operator during the hoisting of building material onto the roof.    Instance 2: Three employees were exposed to a 10.5 foot fall hazard while demolishing existing commercial roof.    PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
