29 CFR 1926.100(a):     Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.        (a) Right Front Doorway - On or about October 29, 2012 an employee was exposed to head injuries while working directly underneath another employee standing on a step ladder. One employee was handling tools and equipment to employee on step ladder. Neither of the employees wore a hardhat.    (b) Front of House - On or about October 29, 2012 an employee was exposed to head injuries while working directly underneath another employee standing on a wooden fabricated scaffold handling tools and equipment. The employees were also passing lumber to one another. Neither of the employees wore a hardhat.

29 CFR 1926.451(b)(2):     Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway shall be at least 18 inches (46 cm) wide.          (a) Front of House - On or about October 29, 2012 two employees were exposed to a fall hazard while working on a homemade wooden scaffold with walkway less than 18 inches.  The employees were standing on a scaffold 9.3 feet above the ground. The wooden scaffold walkway was constructed of one each 2 inch by 11 inch wood board.  The scaffold walkway was only 11 inches wide.

29 CFR 1926.501(b)(2)(ii):     Each employee on a walking/working surface 6 feet (1.8 m) or more above a lower level where leading edges are under construction, but who is not engaged in the leading edge work, shall be protected from falling by a guardrail system, safety net system, or personal fall arrest system. If a guardrail system is chosen to provide the fall protection, and a controlled access zone has already been established for leading edge work, the control line may be used in lieu of a guardrail along the edge that parallels the leading edge.          (a) Top of House - On or about October 29, 2012 employees were exposed to fall hazards while walking and working on the rafter beams. Employees were working 11.5 feet above concrete with no guardrail system or personal fall arrest system.
