Construction Site, 153 Turtle Creek Dr., Hattiesburg MS - on or about November 1, 2012 a common household type extension cord was used to power a MAG 77, 15 Amp skill saw.  The extension cord did not have a grounding prong hole on the female plug and the extension cord outer sheathing was not marked for junior hard service or hard service usage.
