29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replaced: WELDING/MAINT. ROOM; On or about October 29, 2012 the employer did not ensure that the welding lead insulation of an electrical welding machine was free from damage in at least two places. This condition exposed employees to the hazards associated to electrical shock.
