29 CFR 1910.132(d)(2) The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. On October 16, 2012, Huff Salt Water Disposal #32-11, the employer did not certify, in writing, that a hazard assessment of the workplace was done to determine if hazards are present which necessitate the use of personal protective equipment. It was not determined that the personal protective equipment (ppe) provided was sufficient to protect employees from the hazards in the workplace.
