29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.   Abatement Certification and Documentation is required   (a) The 2013 OSHA 300 log  of work related injuries and illnesses was not not sufficient in detail for column E (Where the event occurred) and column F (Description of injury or illness, parts of body affected, and object / substance that directly injured or made person ill).  For example:   1) Log entries numbers RC006087, RC006392, RC006235, RC006262, RC006407, and RC006542 all listed under column E (Where the event occurred) PEWAUKEE PLANT instead of the specific building section, machine number, department or area where the event ocurred.   2) Log entry RC00392 lists an unknown lower back injury without describing a strain caused by extended reaching and leaning while performing jogging; RC006407 lists bone/joint injury to the shoulders involving containers, drums and mail trays instead of the more specific description of lifting 25 pound trays over head to place on a conveyor; and log entry RC006542 list soft tissue injury to the shoulders instead of a more descriptive entry of pinched nerves in the right shoulder from lifting a heavy machine guard to clear jams.
