29 CFR 1910.176(b): Storage of material created a hazard: This violation was discovered on November 05, 2012, in the warehouse area; where the storage of materials on industrial steel storage racks exposed employees to struck-by and crushed-by hazards: a) Base plates on storage rack columns were not anchored to the floor. b) The industrial steel storage racks did not display a plaque indicating the: (1) maximum permissible unit load, and/or maximum uniformly distributed load per level; (2) average unit load if applicable; and, (3) maximum total load per bay.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: This violation was discovered on November 05, 2012, in the de-mold area of the warehouse; where at least one (1) employee worked using an Automated Conveyor Systems Inc., FO No. 219009 conveyor and one or more methods of machine guarding was not provided to protect the employee from hazards such as those created by ingoing nip points, and rotating parts: a) Located between the bearing ends of the tail pulley, roller assembly and the conveyor belt wiper/cleaner, on the number one de-mold line conveyor. b) Located between the bearing ends of the tail pulley, roller assembly and the conveyor belt wiper/cleaner, on the number two de-mold line conveyor.
