The employer fails to assure effective protection against falls as required by the standard, as demonstrated on November 2, 2012, when employees were working on a residential roof at 204 38th Ave. S., Jacksonville Beach, Florida 32250 without fall protection and were exposed to a fall hazard of approximately 25 feet.      Collis Roofing, Inc., previously committed a repeat violation of this standard on June 9, 2012, with respect to a workplace at 2120 White Jasmine Court, Apopka, FL, which was reclassified to a serious violation and became a final order on August 29, 2012 (Inspection No. 472520).
