29 CFR 1910.147(c)(4)(ii):     Procedures did not clearly and specifically outline the scope, purpose, authorization, rules and techniques to be utllized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h):      A.)  C & J Industries - On or about 11/1/12, the company's lockout/tagout (control of Hazardous Energy) procedures lacked items including, but not limited to, specific explaination of how to relieve stored energy,specific procedural steps for locking out and re-energizing the equipment, step for verifying de-energization, steps for notifying affected employees, and the types and magnitudes of energy for each piece of machinery.  Employees perform servicing and maintenance on machinery including, but not limited to, Toshiba Press #75.        ABATEMENT CERTIFICATION REQUIRED:  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement and state that employees and their representatives were informed of this abatement.
