29 CFR 1910.24(b):     Fixed stairs were not provided for access from one structure level to another where operations necessitate regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations:    On or about October 17, 2012, at the operating stations for the Morbark post peelers in the 8 ft. mill, steps were not installed to facilitate travel between the operating platforms and the floor of the facility exposing employees to the hazards of falls.

29 CFR 1910.147(c)(4)(i):     The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules and techniques to be utilized for the control of hazardous energy, including but not limited to Items A-D of this section:    On or about October 17, 2012, specific written lockout/tagout procedures for the eight (8) morbark post peelers failed to clearly identify steps for isolating the pneumatics and the lockout/tagout procedure for the 8 ft. mill morbarks referred to the incorrect main circuit breaker.

29 CFR 1910.212(a)(3)(ii):     The point of operation of machines whose operation exposes an employee to injury, was not guarded:    The employer did not ensure point of operation guarding is place to protect employees from having any part of their body in the danger zone during operation.      a) On or about October 17, 2012, in the 8 ft. mill on the Morbark PS8 post peeler, serial number 372, the debarking mechanism was not guarded exposing employees to being caught in the rotating knives.      b) On or about October 17, 2012, in the 8 ft. mill on the Morbark PS8 post peeler, serial number 522, the debarking mechanism was not guarded exposing employees to being caught in the rotating knives.      c) On or about October 17, 2012, in the West Mill on the #1 Morbark PS8 post peeler, the debarking mechanism was not guarded exposing employees to being caught in the rotating knives.      d) On or about October 17, 2012, in the West Mill on the #2 Morbark PS8 post peeler, the debarking mechanism was not guarded exposing employees to being caught in the rotating knives.      e) On or about October 17, 2012, in the West Mill on the #3 Morbark PS8 post peeler, the debarking mechanism was not guarded exposing employees to being caught in the rotating knives.      f)On or about October 17, 2012, in the North mill on the #1 Morbark PS8 post peeler, the debarking mechanism was not guarded exposing employees to being caught in the rotating knives.      g)On or about October 17, 2012, in the North mill on the #2 Morbark PS8 post peeler, the debarking mechanism was not guarded exposing employees to being caught in the rotating knives.      h) On or about October 17, 2012, in the barn pole mill, the Morbark PS8 post peeler, the debarking mechanism was not guarded exposing employees to being caught in the rotating knives.

29 CFR 1910.219(f)(3):     All sprocket wheels and chains seven (7) feet or less above the floor were not enclosed:      The employer did not ensure the sprocket wheels and chains were enclosed on the Morbark post peelers and supporting equipment such as, but not limited to the following:     a) On or about October 17, 2012, in the 8 ft. mill on the Morbark PS8 post peeler, the chains and sprockets for the spike rollers were not guarded exposing employees to being caught in the ingoing nip point.      b) On or about October 17, 2012, in the 8 ft. mill on the Morbark PS8 post peeler, the chains and sprockets for the spike rollers were not guarded exposing employees to being caught in the ingoing nip point.      c) On or about October 17, 2012, in the West Mill on the #1 Morbark PS8 post peeler, the chains and sprockets for the spike rollers were not guarded exposing employees to being caught in the ingoing nip points.     d) On or about October 17, 2012, in the West Mill on the #2 Morbark PS8 post peeler, the chains and sprockets for the spike rollers were not guarded exposing employees to being caught in the ingoing nip points.      e) On or about October 17, 2012, in the West Mill on the #3 Morbark PS8 post peeler, the chains and sprockets for the spike rollers were not guarded exposing employees to being caught in the ingoing nip points.        f) On or about October 17, 2012, in the North/East mill on the #1 Morbark PS8 post peeler, the chains and sprockets for the spike roller were not guarded exposing employees to being caught in the ingoing nip points.    g) On or about October 17, 2012, in the North/East mill on the #1 Morbark post peeler, the chains and sprockets on the live deck were not guarded exposing employees to being caught in the ingoing nip points.    h) On or about October 17, 2012, in the North/East mill on the #2 Morbark PS8 post peeler, the chains and sprockets for the spike rollers were not guarded exposing employees to being caught in the ingoing nip points.       i) On or about October 17, 2012, in the barn pole mill, the Morbark PS8 post peeler, the chains and sprockets for the spike rollers were not guarded exposing employees to being caught in the ingoing nip points.          JULIAN LUMBER COMPANY WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT 29 CFR 1910.219(f)(3), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314930942, CITATION 01, ITEM 004 AND WAS AFFIRMED AS A FINAL ORDER ON MARCH 29, 2011, WITH RESPECT TO A WORKPLACE LOCATED 10 MILES EAST OF ANTLERS ON HWY 3, ANTLERS, OK.
