29 CFR 1910.134(f)(2):  The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter:    a.) DonJon Shipbuilding & Repair, LLC, 220 East Bayfront Parkway, Erie, PA, on or about November 2nd, 2012, and times prior thereto:  Company employees were not provided with fit tests prior to use of 3M brand half mask air purifying respirators, and annually thereafter.

29 CFR 1910.134(d)(3)(iii)(B)(2): The employer did not implement a change schedule for respirators not equipped with an End of Service Life Indicator that would ensure cartridges were changed before the end of their service life:    a.) DonJon Shipbuilding & Repair, LLC, 220 East Bayfront Parkway, Erie, PA, on or about October 31st, 2012, and times prior thereto:  Company employees routinely use 3M brand half mask air purifying respirators with organic vapor cartridges and particulate filters.  A change out schedule for replacing the cartridges had not been developed.

29 CFR 1910.134(i)(1)(ii):  Breathing air did not meet requirements for Grade D breathing air as described in the ANSI/Compressed Gas Association Commodity Specification G-7.1-1989:    a.)DonJon Shipbuilding & Repair, LLC, 220 East Bayfront Parkway, Erie, PA-Blast Shack-Northwest corner of the facility-on or about January 28th, 2013, and times prior thereto:  Company employees were utilizing supplied breathing air from an oil-lubricated Quincy (QSI-750 Serial # BB1008160130) air compressor through a supplied air respirator, with an Allegro 3 stage inline filter panel system.  The company did not ensure that the compressed breathing air, supplied to company employees, met at least the requirements for Grade D breathing air.

29 CFR 1910.134(i)(5)(iii): The compressor's sorbent bed and filters were not maintained and replaced or refurbished periodically as instructed by the manufacturer:    a.) DonJon Shipbuilding & Repair, LLC, 220 East Bayfront Parkway, Erie, PA-Blast Shack-Northwest corner of the facility-on or about January 28th, 2013, the Allegro Industries three stage filter system (#96424) installed in a compressor line, used to supply breathing air to supplied air respirators, were not replaced as directed by the manufacturer.  The manufacturers instructions stated that the first stage filter must be changed every 200 running hours, the 2nd and 3rd stage filter must be changed every 400 running hours.  The filters utilized to filter supplied breathing air to employees were not maintained and replaced periodically following the manufacturers instructions.

DonJon Shipbuilding & Repair, LLC, 220 East Bayfront Parkway, Erie, PA-Blast Shack-Northwest corner of the facility-on or about January 28th, 2013, and times prior thereto: Company employees were utilizing supplied breathing air from an oil-lubricated Quincy (QSI-750 Serial # BB1008160130) air compressor through a supplied air respirator with an Allegro 3 stage inline filter panel system (#96424).  The companys Allegro filter system did not have a tag containing the most recent change date and the signature of the person authorized by the employer to perform the change.

The company did not monitor for Carbon Monoxide in the supplied breathing air used by company employees who were performing blasting/painting operations.
