Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: This violation occurred on November 6, 2012, at a workplace located at 1050 S. State Highway 161, Grand Prairie, TX 75051; where at least eleven (11) employees, who were working on the roof, close to the edge, were not protected from falling to the ground below.
