This was most recently observed at MCAS Miramar, Hangar 7 where the employer failed to conduct inspections of the worksite that recognized the hazards of aerial lifts conducting work activity near uncovered excavations.

AC17550 - On or about 11/1/2012, the employer failed to inspect and remove from service a fall protection harness worn by an ironworker having burn damage.       b) Guardian harness model no. 01703 - On or about 11/1/2012, the employer failed to inspect and remove from service a fall protection harness worn by an ironworker foreman having burn damage.
