29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:    a) On or about November 6, 2012, the dust accumulation inside the shop on electrical outlets, duct work, electrical cabinets, and conduit exposed employees to fire and explosion hazards.  The employer did not assure work areas such as, but not limited to, Bay 3 was cleaned promptly and to its entirety.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.307(c)(2)(i): Equipment in hazardous (classified) location(s) was not approved for the ignitable or combustible properties of the specific gas, vapor, dust, or fiber that was or could be present:    a) On or about November 6, 2012, the employer did not assure that electrical installations such as, but not limited to, receptacles, electrical cabinets and disconnects, in the Bay 3 woodworking area were Class II rated.  Employees were exposed to fire and explosion hazards from accumulation of wood dust and lack of Class II electrical equipment throughout the shop area.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    a) The employer did not develop and implement a written respiratory program to protect employees from worksite respiratory hazards.   A written program was not developed for employees wearing disposable respirators during wood working and an employee who utilizes a SATA supplied air respirator while performing spray finishing operations in a spray booth. The area in front of the electrical equipment located in the Bay 2 and Bay 3 was blocked by fixtures and/or wood supplies, hindering an employee's ability to access the equipment.
