29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    Location: Facility    On or about 11/2/12, the employer did not establish an energy control program consisting of energy control procedures, employee training, and periodic inspections including but not limited to the employees who clean, change motors, bearings, belts, blades and perform maintenance to the following but not limited to machines;    1. Branson Ultrasonic 2000 cutter machine  2. Back Wrapper machine  3. CNC Bullmer 4501C Premium cut machine

29 CFR 1910.176(b):     Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:    Locations: Paint warehouse    On or about 11/2/12, employees working at the facility were exposed to rack collapse hazards in the following areas;    1. Rack B-34-02-00-01 - the vertical upright was twisted, damaged, bent inwards, and out of plumb by approximately 2-inches.  2. Rack B-33-07-00-01 - the vertical upright was twisted, damaged, bent inwards, and out of plumb by approximately 1 and 7/8-inches.  3. Rack B-30 - the entire rack moved approximately 10-inches inwards at the middle of the rack and was damaged.
