Licensed Specialty Products of Ohio, Inc. located at 130 Cherry St. Bradner, OH; On or about November 6, 2012, the employer did not assure deenergization procedures were utilized when adjusting the Tennsmith Rollers in Step 1 of the wastebasket manufacturing process. Employees were exposed to ingoing nip points when adjusting the one of three rollers while the other two were rotating.

Employees were exposed to ingoing nip points when adjusting the one of three rollers while the other two were rotating.

Licensed Specialty Products of Ohio, Inc. located at 130 Cherry St. Bradner, OH; On or about November 6, 2012, the employer did not assure employees were protected from caught by hazards on the Medium Dryer Line Rollers at the catch area. Employees are required to catch parts coming off the dryer and are exposed to caught by hazards from the unguarded rollers. b.) Licensed Specialty Products of Ohio, Inc. located at 130 Cherry St. Bradner, OH; On or about November 6, 2012, the employer did not assure employees were protected from caught by hazards from the United and Dayton Bench Top Drill Presses. Employees use the drill presses for various milling activities and are exposed to caught by hazards from the unguarded chuck. per ISA added instance c as follows: c) Licensed Specialty Products of Ohio, Inc., located at 130 Cherry St. Bradner, OH: On or about November 6, 2012, the employer did not assure employees were adequately protected from caught in hazards when adjusting roll(s) on the Tennsmith Rollers.

Licensed Specialty Products of Ohio, Inc. located at 130 Cherry St. Bradner, OH; On or about November 6, 2012, the employer did not assure the Richards Electromatic Super Die Maker Saw Trimmer was adequately guarded. Employees are required to use the saw to make new dies and are exposed to amputation hazards.
