29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: (a) On or before October 18th, 2012 employees of R & D Leverage were exposed to caught in and struck by hazards when the guards for two Makino A55 horizontal milling machines were completely removed from the machine while it was operating. The manufacturer placed warning signs on the guards specifically stating not to remove them while the machine was operating.

29 CFR 1910.215(b)(9): Safety (tongue) guards of abrasive grinders were not adjusted to the constantly decreasing diameter of the wheel and the distance between the wheel periphery and the adjustable tongue exceeded one-fourth inch. (a) On, or at dates prior to, October 18th, 2012 employees of R & D Leverage were exposed to caught in and struck by hazards when the tongue guards on several Baldor abrasive wheel grinders were not properly adjusted and were in excess of one quarter of an inch.
