29 CFR 1910.147(c)(5)(i):  Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:      (a)  The inserter operators and leads were not provided with locks or tags to attach to the energy isolating devices, prior to unjamming their machines, as well as cleaning, adjusting, and putting the feeder belts back on the rollers.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):    (a)  Inserter operators and leads unjammed their machines, as well as cleaned, adjusted, and put belts back on the rollers without first isolating the energy sources. 29 CFR 1910.147(d)(4)(i):  Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees:    (a)  Inserter operators and leads unjammed their machines, as well as cleaned, adjusted, and put feeder belts back on the rollers without first affixing a lockout device to the energy isolating devices.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.147(c)(6)(i):  The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed:     (a)  The periodic inspection of the energy control procedures did not include a review between the inspector and each authorized employee, of that employees responsibilities under the energy control program, including observing the authorized employees actually performing servicing and/or maintenance activities on machines or equipment.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.147(c)(7)(i):  The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:     (a)  Training for inserter operators and leads on the energy control program was inadequate, in that it did not include training employees on when and how to lock out the feeder and folder machines prior to unjamming the machines, as well as prior to cleaning, adjusting, and putting belts back on rollers.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.219(e)(3)(i):  Vertical and inclined belts shall be enclosed by a guard conforming to standards in paragraphs (m) and (o) of this section:    (a)  On or about October 3, 2012, the operator of the IN-4 Inserter Line was exposed to the unguarded vertical drive belt on the Feeder.
