29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures: JOB SITE: 12270 McKelvey Hill Drive, Maryland Heights, MO 63043, at this work site, workers were observed on a flat roof of a 3-story apartment complex, approximatley 22 feet eave to ground level, actively removing old roof shingles and were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system or warning line system and safety monitoring system. The workers were exposed to a fall hazard.

The employer allowed his workers to actively tear down flat roof, approximately 22 feet eave to ground level without any form of fall protection. The workers were exposed to a fall hazard.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder: JOB SITE: 12270 McKelvey Hill Drive, Maryland Heights, MO 63043, at this work site, an aluminum extension ladder was used to gain access on and off the roof, approximately 20 feet eave to ground level, positioned on the west side of the apartment complex directly in the rear of the dumpster, and was not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.
