29 CFR 1926.502(f)(1): Warning lines were not erected around all sides of the roof work area. a) Weiland Construction on or about November 22, 2012, at the address of 111 14th St. Toledo, OH: The employer failed to ensure a warning line system was installed around the entire perimeter of the roof deck (work area). Employees working in the area of unmarked/unguarded edges were exposed to falls of up to 17 ft. to the ground below.

29 CFR 1926.502(f)(2)(iv): The rope, wire, or chain did not have a minimum tensile strength of 500 pounds (2.22 kN), and after being attached to the stanchions, capable of supporting, without breaking, the loads applied to the stanchions as prescribed in paragraph (f)(2)(iii) of this section. a) Weiland Construction on or about November 22, 2012, at the address of 111 14th St. Toledo, OH: The employer failed to ensure the material used as a warning line had a tensile strength of at least 500 lb. Caution Tape utilized as the warning line does not provide adequate notice of fall hazards while employees work in proximity to the unguarded edge. Employees working in the area of unguarded and/or insufficiently marked edges were exposed to falls of up to 17 ft. to the ground below.
