29 CFR 1926.453(b)(2)(v):     A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.    a-295 Route 17S, Paramus NJ:  Employees engaged in the operation of an aerial left were exposed to fall hazards of 12ft as personal fall arrest lanyards were not secured to the lift basket. Condition noted on or about 10/20/2012.
