29 CFR 1926.405(b)(1):     Conductors entering boxes, cabinets, or fittings. Conductors entering boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. Unused openings in cabinets, boxes, and fittings shall also be effectively closed.    a.     In the compressor area, there was a 480 volt energized electrical panel which had an unused opening in the  panel thereby exposing employees to a contact hazard.    b.      On the east side of Shaft 1, there was a 2 inch unused opening in the bottom of the energized 120/208 electrical panel thereby exposing employees to a contact hazard.    c.       In the bottom of Shaft 2, there was an unused opening in the bottom of the 277/480 energized electrical panel thereby exposing employees to a contact hazard.    d.       On the south side of Shaft 6, there was an unused opening in the bottom of the energized 480 volt electrical panel thereby exposing employees to a contact hazard.     e.        On the east side of Shaft 6, their was an unused opening in the bottom of the energized electrical outlet box, thereby exposing employees to a contact hazard.    f.        On the north side of Shaft 6, there was an unused opening in the bottom of the energized 480 volt electrical panel, thereby exposing employees to a contact hazard.    g.        On the Neil Street location where tunneling was being performed, there was a unused opening in the electrical outlet box thereby exposing employees to a contact hazard.

29 CFR 1926.416(e)(1):     Worn or frayed electric cords or cables shall not be used.    a.     On the Neil Road where electrical equipment was used to provide power for portable tools and lighting, the electrical cord to the outlet was worn and pulled from the electrical box thereby exposing employees to a contact hazard.

29 CFR 1926.432(b):     Protection against corrosion. Metal raceways, cable armor, boxes, cable sheathing, cabinets, elbows, couplings, fittings, supports, and support hardware shall be of materials appropriate for the environment in which they are to be installed.      a.     In Shaft 2 where electrical equipment was set up to provide power to equipment, tools and other lighting, protection was not provided over the electrical equipment in the shaft to keep water from getting inside the electrical equipment and causing corrosion on the electrical equipment thereby exposing employees to a contact hazard.
