29 CFR 1926.1053(b)(4): Ladders were not used only for the purpose for which they were designed: (a) At the west side of the basement near the Line D wall on or about November 8, 2012, an 8 foot long step ladder was used in the folded up position and leaned against the wall. Employees stood on the fourth rung from the bottom and installed waterproofing membrane on the wall. (b) At the west side of the basement near the Line D wall on or about November 8, 2012, an extension ladder was jerked back from the wall it rested against to allow a roll of waterproofing membrane to roll out to the floor level. The waterproofing membrane was about 3 feet long by 0.5 feet in diameter. The 16 foot long extension ladder was extended to a length of 13 feet and rested against the wall. Employees were on the ninth rung from the bottom of the ladder.

29 CFR 1926.1053(b)(22): Employees shall not carry any object or load that could cause the employee to lose balance and fall: (a) At the west side of the basement near the Line D wall on or about November 8, 2012, a roll of waterproofing membrane, about 3 feet long by 0.5 feet in diameter, was carried while ascending a 16 foot long extension ladder. The ladder was extended to a length of 13 feet and rested against the wall. Employees climbed up to the ninth rung from the bottom of the ladder.
