29 CFR 1926.451(g)(1): The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedure(s) to control or minimize those hazards: On or about November 6, 2012, an employee was exposed to a fall hazard of approximately 18 feet to the dirt ground below, while accessing the roof of the building, to perform an inspection, from a scissor lift; the employee defeated the fall protection of the lift by climbing over the mid rail and top rail of the scissor lift mobile scaffolding.
