29 CFR 1910.157(g)(4): Employees who have been designated to use fire fighting equipment as a part of the emergency action plan were not provided training in the use of appropriate equipment upon initial assignment and at least annually thereafter:    a) Boscovs, 370 Camp Hill Shopping Center, Camp Hill, PA -  Employees designated to use fire extinguishers as part of the employers Catastrophe Preparedness Plan 2012 were not provided training in the use of appropriate equipment upon initial assignment and at least annually thereafter and were exposed to injuries such as smoke inhalation and burns, on or about 11/08/12.      Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

No abatement certification or documentation required.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    a) Boscov's, 370 Camp Hill Shopping Center, Cleaning Room - A 25 foot long orange extension cord used to supply power to a Maytag Performa washing machine used by employees was observed missing a ground pin and the outer sheathing was frayed, on or about 11/08/12.    No abatement certification or documentation required.
