29 CFR 1926.502(d)(6): The employer did not ensure the snaphook was designed to be engaged directly to a horizontal lifeline: On or about November 7, 2012, during roofing activities including but not limited to installing weatherproof shingles, the employer did not ensure all employees on the roof were not engaging the snaphook into the horizontal lifeline. Employees were exposed to a fall hazard approximately 9 feet.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person: On or about November 7, 2012, during roofing activities including but not limited to installing weatherproof shingles, the employer did not ensure all employees on the roof were attached to an individual anchor point capable of supporting 5000 pounds. Employees were exposed to a fall hazard approximately 9 feet.
