29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: On or about 11/06/12, at Monroe Concrete Products Inc. batch plant, the employees were exposed to a fall hazard while working on a platform missing a mid-rail.

29 CFR 1910.24(h): "Railings and handrails." Standard railings shall be provided on the open sides of all exposed stairways and stair platforms. Handrails shall be provided on at least one side of closed stairways preferably on the right side descending. Stair railings and handrails shall be installed in accordance with the provisions of 1910.23. On or about 11/06/12, at the Monroe Concrete Products Inc. concrete plant, the stairways did not have a handrail on both open sides.
