29 CFR 1926.403(b)(1): Examination. The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations: (a). Worksite located at 4219 Highway 169, St. Joseph, Missouri- Employees were exposed to a shock or electroctuion hazard in that the employer had not examined his electrical equipment to find breaches in the insulation, missing ground pins, and the lack of a GFCI.

29 CFR 1926.404(b)(1)(i): General. The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors. (a). Worksite located at 4219 Highway 169, St. Joseph, Missouri- Employees exposed to shock or electroctution hazards were not protected by the use of a GFCI.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a). 29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. (a). Worksite located at 4219 Highway 169, St. Joseph, Missouri- Employees were exposed to a fall hazard in that the laddre being used to access the upper landing surface did not extend 3 feet above the landing surface.
