29 CFR 1910.212(a)(1):  Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc:    a.  Located at assembly #2, employees were required to manually load shock absorbers into crates that were located on a turnstile.  The turnstile would rotate so that employees did not have to reach completely across the crate when loading the shock absorbers.  There was a gap of approximately 5-1/4 inches between the rotating turnstile and a drive motor mount, thereby exposing the employees to an ingoing nip point.        American Showa was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 313519803, citation number 1, item number 1 and was affirmed as a final order on August 25, 2009, with respect to a workplace located at 960 Cherry Street, Blanchester, Ohio 45107.

You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.    a.  Located at America Showa in Sunbury, Ohio:    a)  On the 2012 OSHA 300 log, the employer did not include all information that was required in Column F.      1.  Case number 48 does not include which foot was sprained when caught in the conveyor.      2.  Case number 52 does not include which wrist was sprained from rolling a part.      3.  Case number 53 does not include which leg developed pain at assembly number 3.      4.  Case number 54 does not include which hand and wrist was affected from the repetitive motion.      5.  Case number 55 does not include which specific body part was affected when removing a tub.      6.  Case number 56 does not include which foot was injuried by a tugger.      7.  Case number 57 does not include which knee was injuried when the employee slipped in an aisle way.     8.  Case number 58 does not include which shoulder was injuried when an employee was pushing 6R machine.     9.  Case number 59 does not include which wrist was lacerated from removing a jig from a cart at #3 assembly.
