(a) On, or at dates prior to, October 17th, 2012 employees of Schulze tool company were exposed to caught in and struck by hazards when the tongue guards on a Chicago Power Tool Company abrasive wheel grinder was not properly adjusted  and was in excess of one quarter of an inch.
