29 CFR 1910.215(b)(9): Safety (tongue) guards of abrasive grinders were not adjusted to the constantly decreasing diameter of the wheel and the distance between the wheel periphery and the adjustable tongue exceeded one-fourth inch. (a) On, or at dates prior to, October 17th, 2012 employees of Schulze tool company were exposed to caught in and struck by hazards when the tongue guards on a Chicago Power Tool Company abrasive wheel grinder was not properly adjusted and was in excess of one quarter of an inch.

29 CFR 1910.212(a)(2): Machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points and rotating parts: (a) On October 17th, 2012 employees of Schulze Tool Company were exposed to caught in hazards when a portions of two separate vertical band saws were not properly guarded. No adjustable guards were in place on the band saws.

(a) On October 17th, 2012, employees of Schulze Tool Company were exposed to an electrical hazard as a flexible cord (extension cord) was secured to the building structure from an electrical outlet in the welding area and used to permanently power an overhead light.
