29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers. At the site where workers were performing plumbing and HVAC installing while walking and driving scissors lifts in the viciinity of floor holes, employees were exposed to potential tripping and tipping hazards.
