29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: On or about November 7, 2012, and times prior thereto, the employer failed to employ safety-related work pratices to prevent electric shock or electrocution resulting from either direct or indirect contact with energized parts while employees preformed the task of changing 1000 watt light bulbs in a 480 Volt high bay lighting system.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: On or about November 7, 2012, and times prior thereto, the employer failed to provide and require the use of appropriate electrical protective equipment for employees involved in relamping activities. Employees were exposed to the hazard of electric shock and/or electrocution.
