29 CFR 1926.95(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    (a)  At the project site on or about November 9, 2012, and at times prior thereto, hand protection was not worn while applying 3M Scotch-weld Neoprene Contact Adhesive onto the underside of counter covering material.  The MSDS stated avoid skin contact.  MSDS also recommended Butyl Rubber, Polyethylene/Ethylene Alcohol gloves.

29 CFR 1926.404(b)(1)(i):  Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.    (a)  At the project on or about November 9, 2012, and at times prior thereto, there was no ground-fault circuit-interrupter protection or an assured equipment grounding conductor program when a 50 foot extension cord was used to power a 110V AC DeWALT angle grinder, model D28402.  The angle grinder was used to cut sheet metal.

29 CFR 1926.404(f)(6):  The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    (a)  A the project site on or about November 9, 2012  and at times prior thereto, an employee was using an extension cord with a missing ground pin to provide temporary electrical service to a  DeWalt angle grinder, model D28402.  The angle grinder was used to cut sheet metal.
