29 CFR 1910.303(b)(7)(iv):     There were damaged parts that could adversely affect safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.   a) On or about 11/8/12 in the Maintenance shop; a yellow cord set (extension cord) was found insulation pulled away from the male end and being used to supply power to portable electrical equipment, such as but not limited to, saws, lights, power tools.      b) On or about 11/8/12 in the Janitor area; a red cord set ( extension cord ) was found with insulation pulled away from the female end and being used to supply power to portable electrical equipment, such as but not limited to, wet dry vacuum.
