29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a. Site- Employees installing the top half of the roof trusses on the entry way and donation drop off dormers were exposed to a fall of approximately 25 feet where adequate fall protection was not provided by the employer.    b. Site- Employees exiting the baskets of aerial lifts to access the flat roof were exposed to a fall of approximately 15 feet where adequate fall protection was not provided by the employer.    c. Site- On or about 11/2/2012 an employee was exposed to a fall of approximately 25 feet when they exited an aerial lift, climbed the roof face of the entry way dormer to the top chord of the lower half roof truss, retrieved a rope while walking on the top chords of the roof trusses, and then slid down the sheathed roof face to the basket of the aerial lift.
