29 CFR 1910.147(c)(4)(ii):  Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):      a.  On or about 11/5/12, at Elore Enterprises LLC.  Employees were exposed to caught by and electrical hazards while working, cleaning, sanitizing, servicing and maintaining equipment for which documented lockout/tagout procedures lacked of, among other conditions; a specific statement of the intended use, specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them;  specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures, etc.

On or about 11/5/12, at Elore Enterprises LLC.  Energy isolating devices were not available for employees while performing work requiring their use (i.e. cleaning, sanitizing, servicing and/or maintenance requiring bypass of guards or safety devices), exposing employees to caught by and electrical hazards.

29 CFR 1910.304(g)(5):  The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective.    a.  On or about 11/9/12, at Packaging #2, Elore Enterprises LLC.  An employee was exposed to the risk of receiving a minor electrical shock while working with a packaging machine that was energized by using an extension cord that lacked a ground prong on its attachment plug.
