Richie Artesia, NM, Building 30, a Clark power industrial truck was used with the back-up alarm removed. The backup alarm was provided as part Clark's original power industrial truck equipment.

29 CFR 1910.178(q)(7): A powered industrial truck was not examined before being placed in service, and was placed in service when there were conditions that adversely affected the safety of the vehicle: On November 08, 2012, and times prior thereto, at 1300 N. Richie Artesia, NM, Building 30, a Clark C15 powered industrial truck was being used without prior examination. The vehicle did not have a functional horn, and the back-up alarm had been removed, exposing employees to a struck-by hazard.
